Fetch a patient care plan

Fetches a patient care plan enrollment by its ID.

A patient care plan represents a specific patient's enrollment in a care plan template. It tracks the enrollment status, billing details, start and end dates, and renewal configuration. A single patient can be enrolled in one core plan and multiple add-on plans simultaneously.
