Apply Location Import

Apply this import: create net-new locations, update the ones that changed, and take stale ones off the roster.

Takes the same file and mapping as the preview rather than a diff posted back from it, so the plan is recomputed here by the same planner and the caller cannot choose which rows get rewritten. Rejected with a 400 while the file still has validation errors.
