Set a subject's maximum mark

The maximum belongs to the subject, not the candidate, so it is set once and applied to every candidate holding that subject in that year — one statement, not a loop. Every updated row shares a batch id and gets a history row, so a mis-typed maximum is one thing to find and reverse.
