Create Chain

Register a chain and dispatch its no-dependency steps.

Idempotent within 30s: caller may retry on transient failure (504, network drop) without producing duplicate chains. Implicit dedup key uses (user_id, body JSON); explicit Idempotency-Key header overrides.
