Password Reset Request

Issue a 6-digit password-reset code via email.

Always returns 200 with the same generic message regardless of whether the email belongs to an account, to avoid leaking which addresses are registered. A pending lockout (pwd_reset_lock:{email}) silently suppresses code generation.
