Create Definition Instance

Add another account instance for a connector definition.

The ordinary Connections UI sends personal=true. That creates one owner-held account in the caller's home folder and accepts no routing or mode override. Administrative and MCP callers retain the explicit connection_mode, destination and target-identity contract.

The instance reuses the selected or recommended connection profile and starts unconnected. The owner completes OAuth or credential setup from the connection page.
