Read from the platform ledger, not fanned out to the venues; each row is annotated with the `deploymentId` that owns it, or none if placed by hand. A manual order the exchange refused at placement never received an exchange order id: its row has `id: null`, keeps `clientOrderId`, and carries `placementRejected: true` (scope: trading:read)
