Resolve Direct

Resolve target adapter and mint a signed direct-link token for the authenticated caller.

The caller is the verified consumer credential, never a body field (REQ-404). The upstream URL is returned on purpose: a direct link bypasses the gateway, so it is handed only to an authenticated consumer. Public discovery (GET /v1/discovery) never carries it.
