Set one RBAC policy row for an arbitrary (group, individual)

Changed on

Append-only write of one (group, ind, permission, enabled) tuple. Enforces the last-permissions-manager guard and confirms the target individual is currently a member of the group. Caller must hold permissions_manage on the active group in legacy mode. In enforced mode, admission requires roles_manage, then this permission-row write is refused with 409 authorization_mode_enforced.
