Resume planning under the same durable StoryPlan id.

State-aware and replay-safe. Ordinary PENDING plans repair a missed queue dispatch, stale RUNNING plans reclaim a dead worker, and FAILED plans without generated output re-enter generation under the same id after clearing their failure. A PENDING plan with failureCode: provider_capacity_backoff already has a Sleeper Hit-managed delayed retry and is returned unchanged. Active, review-ready, and approved plans are also unchanged. REJECTED and SUPERSEDED plans remain human-terminal and cannot be resumed. Requires Idempotency-Key.
