Org Policy

Add a policy entry

Adds a skill to the org's policy (its effect depends on the policy mode). Org-level entries require the ADMIN org role; department- or team-scoped entries are part of the governance preview and require a platform admin. Idempotent per scope; recorded in the audit log.
