Approve a pending approval request.

Writes a signed .approved decision record (source web, attributed to the authenticated principal, bound to the open request's nonce) so the orchestrator poll loop unblocks. The pending file is then removed.

Args: request: The HTTP request; its credentials name the principal. task_id: Task ID to approve. body: Optional reason metadata.

Returns: Success message.
