Approve a profile suggestion and apply it to the coach profile

The human approval that lets ingested content reach the profile. It writes through the same path a direct profile edit uses, and re-checks the field against the allowlist instead of trusting the stored row. Approving a suggestion that's already resolved returns 409.

API key scopes: knowledge:write. You also need an Idempotency-Key header.
