Publish channel instructions

Publish a new version of the channel's CONTEXT.md instructions. Pass base_version (the version you read) so a concurrent edit is rejected with 409 instead of overwritten.

base_versioninteger nullable

Optimistic-concurrency guard: the version the edit is based on (0 for a channel with no instructions yet). A stale base is rejected with 409; omit to publish unguarded.
