On-demand subtitle generation (lazy cache) for a story OR storyverse episode.

Flow: check subtitles table → return cached URL if present → otherwise download video, generate for requested language_code, upload VTT, save metadata, return URL.

Public content: any viewer can request generation. Private stories: owner auth required.
