Complete Onboarding

Finalize first-login onboarding: set bot name + mark as onboarded.

Idempotent — calling after onboarding still updates the name if unchanged, but won't reset onboarded_at or convert the durable execution mode. The hosted/byo choice becomes server-authoritative on first completion.

Handles the TOCTOU race where two concurrent requests pick the same name: the DB unique constraint on agents.name raises IntegrityError on commit and we translate it into a 409 without crashing.
