The durable review payload for one agent-touched file

The persisted review content for one touched (path, runId): for a content op (write / edit / delete) the captured pre/post snapshot under mutation (the same field set the per-session stream's fileMutation frames carry — null content with a false omitted flag means genuinely absent on that side, a true flag means uncaptured and the reviewer re-reads current bytes); for a move / rename the {from, to} origin summary under move (null when the origin was never captured). This is the review panel's content source: unlike the per-session GET /v1/agent/stream ring it survives the ring rolling, the 64-session LRU, and a ToolPiper restart. {found: false} for an unknown or already-cleared touch — the reviewer falls back to a current-bytes re-read.
