Record verified operator reconciliation for an expired unknown order

Stop and fence every perp worker, verify the exact venue order is no longer live, and reconstruct its final fills. Only expired unknown orders with no active lease can be resolved. This records an operator attestation, not a venue confirmation. Previously recorded fills cannot decrease. The same owner and venue grant are required; retry with the original idempotency key.
