Revoke a team-admin grant

Authorized: org_admin / org_owner (or the grantee revoking their own active grant on this team). Authorization: team_id is resolved to its owning organization from the DB and the caller must be an owner/admin of that org — or be user_id themselves revoking an active self-grant on this team — so a team in an org the caller does not belong to returns the same 404 as a nonexistent team (a member with a lower role receives 403) and no other tenant's grant can be revoked.

Timestamp of revocation. Present only when revoked: true.

Actor who revoked the grant. Present only when revoked: true.

true when there was no active grant to revoke. revoked is false in this case.
