List an org's active OAuth grants (connected apps / sessions)

Lists the live OAuth grants (unrevoked, refresh-token-unexpired) issued against an org — the owner-facing 'Connected apps / active sessions' view. Returns only non-secret metadata: client, scopes (including per-agent resource scopes), issued/expiry times, and a best-effort last-used stamp — never any token material or hash. Caller must be an owner or admin of org_id. Optionally filter to grants connected to a specific agent with agent_id.
