List buyer groups

The buyer groups of a campaign that distributes by buyer groups, in the order leads go through them, each with its buyers in order.

A group's id is what buyer_group_id takes when creating or updating a buyer. A buyer in no group is not listed and receives no leads.

A campaign that distributes some other way answers 422. Groups are read only for now: they are created and edited in the application.
