Capability Health

T2: per-capability self-monitor. Actively probes each capability and reads background-scheduler heartbeats, so a capability whose loop silently died (the K2 dead-code class of bug) surfaces as down/stale instead of hiding behind a process-level 200. Serves the 'one integration, system watches itself' promise.
