List recent OTel ingest failures, grouped by signal and error class.

The companion to /otel/pipeline-stats: that endpoint reports how many records were dropped, this one reports why, so an operator can tell a ClickHouse outage from a schema mismatch without reading server logs.

Read-only and system-scoped (no project parameter), matching get_pipeline_stats — the counters it explains are process-wide.
