Revoke a verifier

Sets status="revoked" and revoked_at for caller-owned scope=user verifiers only (UUID or name). Platform scope=system verifiers reject as 404. The verifier disappears from list/get/run for the owner; existing run rows are retained. Irreversible; replace by deploying a fresh verifier under a new name.
