Update Payor Credentials

Upsert the per-payor portal configuration: credentials plus billing config.

totp_secret is the portal's authenticator (TOTP) base32 seed; the operator generates the rotating 2FA code from it at login time. billing_provider and service_line are the non-secret per-payor defaults merged into every task's params.
