Disconnect Personal Connector

Sever the caller's own connection. Never feature-gated: even when an admin has turned the feature off, users can still take their access back.

The mirror row is deleted only after Nango confirms the connection is gone — it's Dot's only pointer to the grant, so deleting it on a vendor outage would leave a live token nobody could ever revoke.
