End-of-session heartbeat coverage report

heartbeat_coverage_pct measures credited heartbeat/session-enforcement time divided by cumulative joined time. It is not Focus Alignment. in_focus_pct is deprecated. blocking_permissions_enabled is the latest explicit client report (null when unknown), not historical permission coverage. opted_out_count counts declined students and students without blocking permissions once.
