Verify TOTP code and activate 2FA. Returns recovery codes.

When called with a setup_token (purpose=2fa_setup, issued at login for admins
who haven't configured 2FA yet), also emits a full access_token + refresh_token
so the frontend can log the user in immediately after setup.
