Relay Self Confirm Endpoint

Step 2 of the self-submit rail: register the rating the rater sent.

Everything is read from EM's record and from the chain; the request only names the transaction. Pending is 202, never 409 — a 409 on this rail means "already rated" and a sweep acts on it.
