Merge Exercises Into One

Merge several teacher exercises into a single one.

Used by the "mes exercices" import flow: when an upload is parsed into n>1 exercises and the teacher chooses to keep them as one. Each source figure is preserved as an inline figure://id marker in the merged content (Figure docs are NOT deleted — only the source exercise wrappers). The source exercises are removed afterwards.
