Accept Invitation As New User

Accept an invite as a brand-new user (unauthenticated — the emailed token is the proof of address control). Creates an already-verified user + their membership + marks the invite accepted, all in the one request transaction, then auto-logs-in (token scoped to the joined tenant, set as their default). If a user already exists for the email, it's a 409 → use the sign-in flow.
