Stream Instructions
Changed onSSE endpoint for streaming instructions to agents for a specific session.
Uses PostgreSQL LISTEN/NOTIFY (channel agent_instructions, payload = session_id) instead of polling: a dedicated asyncpg connection holds a LISTEN, the main loop awaits the next notification or a heartbeat timeout, and only queries the database when a notification arrives or on initial connect.
