EDGE-100 MCP Gateway skeleton — upstream forwarding (disabled by default)

Catch-all for /api/v1/mcp/gateway/upstream/*. Always returns 503 in the P1 skeleton: gateway_disabled when MCPPolicy.GatewayEnabled is false (default), no_upstream_configured when true but the EDGE-101 upstream registry is empty.
