Merge a duplicate patient account into the account to keep

Changed on

Moves every appointment, payment, chat, questionnaire, note and log of the duplicate to the kept account and deactivates the duplicate, sign-in included. Where both accounts hold the same unique record, the kept account's one wins. Not reversible.
