List Hunches

List the user's hunches with their latest evaluations.

status=active returns the Tracked lane (monitored hunches only); resolved the graduated shelf (learned/unsupported); all both. Proposed hunches (agent-staged, unconfirmed) are never returned — they live entirely in the MCP conversation until confirmed.
