Qbo Callback

Handle QBO OAuth callback.

Intuit redirects here with ?code=...&state=...&realmId=... after the user authorises the app. We exchange the code for tokens, fetch CompanyInfo to resolve the company name/country, upsert the QBOIntegration row, and bind the integration to the claim that initiated the flow.
