Record a person Rule 506(d) covers

Adds a 20% holder, promoter or compensated solicitor to the round's bad-actor list, or records anyone's questionnaire: the date, the answer as given (all_no or some_yes) and the signed questionnaire. Directors and executive officers on record are listed already; pass party_id to record theirs. Binderwork stores the answers and never says whether anyone is disqualified.
