Bulk Create Agents

Bulk-register agents from CSV / a registry export (RES-009 §X #13).

Each row is processed independently: a duplicate name in row 7 doesn't block rows 8..N. The response gives a per-row status with the original index so the UI can render line-numbered errors next to the upload preview.

Duplicates are reported as "skipped_duplicate" rather than an error because re-running the same CSV is a common workflow (re-import after a fix in row 12 shouldn't fail rows 1-11 that already exist).
