Affiliate Admin

Record one manual payout for one affiliate (spec 4.6).

<br/>v1 pays by hand: the admin has already sent the PayPal or bank transfer<br/>and is recording it with its reference, so the payout is created paid<br/>and every commission it covers flips to paid in the same transaction.<br/>A balance below PAYOUT_MINIMUM_CENTS, negative balances included, is a<br/>422 that writes nothing and rolls into the next run (R11).
