Record an Approved Expense (Template Sec E).

Participation Agreement Sec 8.1 ("No Surprise Spend") and Exhibit A A2: no expense is billable to an Owner without written pre-approval. approval_reference is therefore MANDATORY -- an expense with no recorded approval is not an Approved Expense and must not be able to reach an owner's statement.
