Clear the account's avatar

Remove the account's picture, so GET /organisations/{org_id}/avatar answers 404 and a console falls back to whatever it draws for an account without one. Gateway-native. Account admins only (settings_manage). Idempotent: an account with no picture answers 204 unchanged. The stored bytes are content-addressed and may be shared, so they are not reclaimed here. The org_id in the path must be the caller's token org: another account's id is a 404.
