Export the monthly per-application usage report as CSV

Returns the same daily per-application HWM data as usage_report/apps.json, flattened to one CSV row per resource type per application per day, with the application's cloud and its readable source and target endpoints. A resource shared across applications appears under every application that uses it. Source and target reflect each application's current endpoints.
