What to ask a doctor for, country by country.

Public and tokenless: it is a form schema, identical for everyone, and the wizard needs it before a token is necessarily in hand. Fetched once at mount rather than per keystroke — the whole table is a few kilobytes and a round trip per country change would make the form feel broken on a plane.
