Post Initial Tier Override

Coordinator override of the algorithmic initial tier.

Per PRD §13.13, the override changes current_tier but leaves the initial_tier_was_hard_escalator flag unchanged so the pre-op re-tier sticky-hard guard still respects the underlying clinical condition.
