Nephro Setup

Prepare (and with activate=true, open) the reusable nephro campaign.

Same repeatable helper as scripts/setup_nephro.py --apply [--activate]; reruns never duplicate the project, template, campaign or rate, and activation is refused while the readiness report lists blockers.
