Agent Insert Slide

Add a new slide to a draft/review version, right after after_key (default: at the end).

Creates the deck_slide row (next free number, or slide_key when given), writes its first revision from head_extra/css/body/js_json and places it in the manifest. Existing slides keep their keys and URLs. Not for subset decks: their slides come from the source deck.
