Per-video ARV document

Everything ingestion stored for one video, as one document: chapters, shots, visual windows, entities, timed transcript cues when the creator publishes them, and the evidence lists (speakers, audio_spans, people_spans, products, places, labels). Text and times only: no stream URL and no vector. A video the rights policy does not let crawlers search answers 404.
