Upload Entity Image Endpoint

Upload a profile or cover image for an entity. Permission-gated to the claimed owner or a platform admin (same rule as PATCH).

On success returns {ok, url, kind} AND atomically writes the URL to the entity row so the next /entities/{slug} read sees the new image without a refresh.
