Create Guardian Invite

Minor on hard-block kind triggers guardian invite. We attempt the linked-guardian path FIRST (Option C from chapter audit): if minor has guardian_user_id set, status='pending_invitation' goes to that guardian directly via inbox. Otherwise we fall back to email/phone invite-by-link.
