Create Access Request Endpoint

Request access to the tenant that owns the caller's verified email domain.

Idempotent: a repeat POST while a pending request exists returns the same operation envelope without creating a duplicate row or notification. A dismissed request allows a fresh re-ask (rate-limited to 3/day, NFR-002).
